The death of Filipino nationals in Morocco, Guinea-Conakry, Mauritania, Mali, and Senegal should be reported to and registered with the Philippine Embassy in Rabat.

1. One (1) duly accomplished Report of Death Form
2. Copy of passport data page
3. Original Death Certificate (with English translation)
4. Original Embalmer's Certificate (with English translation)
5. Original Certificate of Cremation (with English translation) – for cremated
remains
6. Photocopy of Birth Certificate
7. Photocopy of Marriage Certificate (if applicable)
ADDITIONAL REQUIREMENTS (if remains will be shipped in the Philippines)
1. Original Transit Certificate (with English translation)
2. Original Non-Contagious Disease Certificate (with English translation)
3. Information Sheet for the Shipment of Remains
4. Letter of Acceptance of Human / Cremated Remains
5. Consular Mortuary Certificate – to be issued by the Philippine Embassy
